Wellness Blueprint

Our recommended starting experience.

$379 per month for three months a total of $1,137.

Begin with medical care, coaching, and follow-up in one three-month program. Your Blueprint includes an initial Integrative Medical Evaluation, prerecorded lab review videos as needed when labs are ordered, one follow-up medical appointment, and two Support Coach sessions.

Integrative Medical Evaluation

$579 for your first comprehensive medical appointment.

Meet with Dr. Casiano for up to one hour to discuss your history, current concerns, and goals. If labs are ordered, prerecorded video reviews of your results are provided as needed. Your evaluation also includes two weeks of patient portal support for questions that clarify your appointment recommendations.

This option allows you to begin with one appointment and decide on additional care afterward. Lab testing and any later appointments are billed separately.

Book an Integrative Medical Evaluation

Functional Lab Review

$229 for a focused 30-minute physician appointment.

Take a closer look at recent routine lab results you already have. Dr. Casiano explains relevant findings, considers patterns, and answers your questions through an integrative medicine perspective. Please submit your labs at least three business days before your appointment so they can be reviewed in advance.

This focused service does not replace a comprehensive initial evaluation. If your concerns need a broader review, our team can help you choose an appropriate starting option.

Starting at $29 per month for a month to month membership.

Explore wellness education through webinars, the MedEd resource library, and other member benefits. Discover and Connect do not require a medical evaluation to join and do not include individualized medical care or medical advice.

A closer look at the Wellness Blueprint

The Wellness Blueprint gives you a supported start, whether you are taking preventive steps or seeking help with ongoing health concerns. Across three months, you will meet with Dr. Casiano, review your findings and recommendations, and work with a Support Coach to begin putting your plan into practice.

What your Blueprint includes

One initial Integrative Medical Evaluation with Dr. Casiano.
Prerecorded lab review videos as needed when labs are ordered.
One follow-up medical appointment to discuss findings and recommendations.
Two Support Coach sessions to help you begin applying your plan.
A customized health plan and personal nutrition guidance.
Secure portal message support, medical handouts, a monthly newsletter, and access to eligible in-person Arizona classes.
Care coordination and collaboration with your primary care provider when appropriate, personalized physician research, and priority scheduling.

Additional benefits

During your active Blueprint, the lab administration fee is waived. You also receive 25% savings on eligible added medical follow-up and coaching appointments, 40% on eligible added classes, and 10% on eligible supplements. Lab testing and supplements are separate purchases.

Your investment and pay-in-full bonus

The Blueprint is $379 per month for three months, for a total of $1,137. Total Member Value: $1,390.96.

Pay in full to receive a $100 Wellness Bonus toward eligible labs or supplements. This bonus can help with the cost of eligible purchases. The bonus does not reduce the program price and applies to eligible purchases during your active program under the offer terms.
